The Case of Continuous Saline Irrigation Dressing in Chronic Wound

Abstract
Chronic wound problem has been a complex difficulty in wound management especially in persons with chronic wound infection. Today, the rate of traffic accident is increasing. It results in increasing the rate of open lower leg fracture. Most open tibia fracture is often accompanied by a severely crushed soft tissue injury. It causes the vasculature of the crushed soft tissue to be severely compromised. As a result, an acute open tibia fracture with crushed soft tissue injury is easily converted into a chronic wound, furthermore developed into a chronic osteomyelitis. So the most important treatment modality for the open tibia fracture is early soft tissue coverage. For 51 years old female patient, we tried to cover the chronic open tibia injury with a bipedicled flap. But the opening of the wound was redeveloped due to internally remained inflammation and infection. I tried to treat it with a continuous saline irrigation dressing. The final result was satisfactory. In this study, I can find that the continuous saline irrigation dressing is a novel alternative for the treatment of chronic open wound in the situation with few choices.
